As previously communicated, all TriHealth team members, clinical and non-clinical, are required to get a flu vaccination by Friday, November 12, 2021. Exemptions will be considered and made for documented medical reasons or deeply held religious beliefs. Medical and religious exemption forms for the flu vaccine and supporting documentation are due by Friday, October 29, 2021. If you feel you have a medical and religious exemption, please submit your exemption forms for both requests.

If you have received a medical or religious exemption for the flu vaccine in prior years or the COVID-19 vaccine this year, you must still complete the process this year to have an approved exemption for the upcoming influenza season. As a reminder, the deadline for submission of medical and religious exemption forms for the COVID-19 vaccine expired on September 17, 2021.
