It's now easier than ever to submit your biometric screening results for credit on your Healthy Living Scorecard.

If you complete your biometric screening at a TriHealth primary care physician's office, use MyChart to access, print and fax your results to the LifeStyles Department.

Here's how:

• Log into MyChart (First-time users must get log-in information and instructions from their physician's office.)
• Navigate to the Healthy Living Scorecard section by clicking on My Medical Records > Health Trends > Healthy Living Scorecard Report.
• Print your report and fax it to 513 852 7441 with your team member ID.

Note: If you receive a biometric screening from a LifeStyles staff member, you do not need to submit your results; they're automatically added to your scorecard.

A biometric screening is one required component of the scorecard, which allows team members to earn a discount on TriHealth insurance plan premiums.
