Each year the National Patient Safety Foundation (NPSF) takes the lead in promoting patient safety through National Patient Safety Awareness Week.
A patient's health journey often starts with diagnosis, but experts estimate that up to one in every 10 diagnoses is wrong, delayed or missed completely and that, collectively, diagnostic errors may account for 40,000 to 80,000 deaths per year in the US.
TriHealth would like to take this opportunity to recognize all associates, physicians and leaders for their continued diligence to ensure the safety of TriHealth patients.
Good Catch Program Kick-Off
In addition, the Patient Safety department is kicking off TriHealth's Good Catch Program.
The program recognizes an individual who identified and reported an incident in which harm or potential harm to a patient was prevented or averted.
Each month, two TriHealth associates will be recognized for reporting a near miss incident or "good catch" in the safety event reporting system (currently known as Quantros). We will award the first winners for January and February during National Patient Safety Week.
Winners will be awarded a Good Catch certificate and a $50 gift certificate for TriHealth merchandise at https://trihealth.josephbeth.com/. So, report near misses and you'll be eligible to be a Good Catch awardee!
New Name for Event Management System
Thanks to everyone who voted on or submitted a name for the new incident reporting system! By overwhelming vote, the chosen name for TriHealth's new reporting system is IRIS - Incident Reporting Information System.
Representatives are on site at TriHealth this week to start the design and configuration of IRIS. Look for more information in the coming weeks.
