Effective Oct. 1, TriHealth Parish Nurses will be renamed TriHealth Outreach Ministries. This name change more clearly reflects this department’s mission and services.

There are two important ways Outreach Ministries and our nurses, social workers and community health workers can help underserved and at risk patients.

Outreach nurses provide services such as support for patients recently discharged from the hospital; referrals to various services such as pre-natal care, grief support and other programs; and spiritual support.

Community health workers work to improve infant survival, providing pregnancy and parenting education and support.

Social workers work with both groups to ensure patients have access to all the services they need.
