In mid-May we completed our second annual TriHealth Safety Summit, and today ushers in the first full week of June, which is "National Safety Awareness Month." So I am devoting this Weekly Update to sharing a progress report on TriHealth's safety, quality and high reliability journey, which will ultimately enable us to become the safest and highest quality health system anywhere!
"First, do no harm." This most important tenet of the Hippocratic Oath also serves as the rallying cry for our safety and quality journey - a journey defined by our ability to "Lead the Way to Zero Harm." Just over one year ago, we began this crucial system-wide work to ensure that the safety of our patients and team members is ALWAYS our highest priority. In May of 2018, we launched our first annual TriHealth Safety Summit as part of our Spring LOI, where we embarked on our High Reliability Organization (HRO) journey with nearly 1,000 clinical and administrative TriHealth leaders. Since that time, we have made great strides and are seeing encouraging progress, thanks to the dedication, discipline and hard work of all of our leaders, providers and team members. Following are just a few of our Year 1 HRO Highlights:
- Trained more than 12,000 system team members and leaders, and over 1,200 providers on safety and reliability tools and best practices.
- Created the TriHealth Institute for Safety, Reliability and Clinical Performance Improvement.
- Added resources and talent in Lean, infection prevention, cause analysis, occupational safety and safety leadership to support and sustain our system-wide patient safety efforts: Jeff Norton, VP, TriHealth Institute; Paul Yates, Infection Prevention Officer; Ginger LaMar, Director of Operations, CMO Office; Doug Fuller, Manager, Clinical Analytics; and Michael Waterman, our new Lean Director who joins TriHealth on June 17.
- Introduced and hardwired daily and weekly safety huddles for work groups, teams and departments across the system to improve communication and to anticipate and address possible safety concerns.
- Currently rolling out approximately 800 High Reliability Coaches to help implement and more deeply hardwire HRO practices across the system.
This foundational work is already yielding impressive improvements and external recognition, including:
- 65+ lives saved through adoption of our new sepsis protocol.
- ~15% reduction in hospital-acquired infections, resulting from Lean process improvements and use of new technology introduced by clinically led task forces.
- 20% decrease in patient falls with injury due to local/unit level focus on fall prevention tactics and communication, coupled with introduction of a new predictive analytics tool.
- Achieved Top 5th %tile in U.S. (from ~85 th %tile to >95th %tile) for risk adjusted mortality at both Bethesda North and Good Samaritan, resulting in hundreds of lives saved when compared with "average" hospitals!
- All 4 TriHealth general acute care hospitals received ·A· Hospital Safety Grades in most recent Leapfrog Survey - only 1 in 3 U.S. hospitals achieve "A" ratings!
I want to emphasize that while our journey is supported by technology, tools and best practices, sustainable improvement in safety does not happen without YOU - our providers, leaders and team members who insist on making Every Day a Safety Day at TriHealth! As part of our journey to become a High Reliability Organization, we are in the process of deploying a technology aimed at preventing secondary infections like MRSA and c. diff in our patients and team members. Team Members Give Back to the Community During Healthcare Appreciation Week
As part of Healthcare Appreciation Week, TriHealth team members chose to give back to the Cincinnati community that we serve-partnering with the Freestore Foodbank to donate two apples from each team member to the children who are part of free and reduced lunch program. Read more on Bridge
RaNae Wright Named Director of Patient Experience
RaNae Wright has been named TriHealth's new full-time Director of Patient Experience. In her role, she and her team will be responsible for supporting our leaders and team members at all levels across the system in our work to deliver an unparalleled customer experience. Stroke Recognition & Survival Assigned in TriHealth Learn
The course Stroke Recognition and Survival - B.E.F.A.S.T. has been assigned to all team members, including providers, nonclinical and clinical, at the recommendation of The Joint Commission following their disease-specific survey at Good Samaritan Hospital. Access and complete the course in TriHealth Learn on or before July 1, 2019.
