Jayne Miller has been promoted from manager of TriHealth Navigator for Senior Services to General Manager of TriHealth Navigator. From the creation of Navigator for Senior Services to the development of the new Care Transitions Navigator initiative, Jayne has championed the evolution of the Navigator concept.
Through her leadership and collaborative efforts, the program has grown to encompass the provision of complex care management for our community, our patients and TriHealth team members, integrating best practices to address continuity and readmission issues across the TriHealth system.
Jayne's work with various TriHealth services lines and institutes exemplifies the type of integration that will lead us into the future as a unified system. The enhanced coordination of care made possible by our Care Transitions Navigator program will assist TriHealth in further lowering readmission rates and facilitating improved outcomes for our patients. Jayne's efforts and continued vision for TriHealth Navigator have led to this most recent achievement in her 28-year career with TriHealth.
