TriHealth Bridge

June 27, 2014

Despite the recent decision not to continue the TriHealth SeniorLink program, our health system remains dedicated to serving the needs of the growing aging population in Greater Cincinnati through TriHealth Senior Services.

Combining the strengths of our hospitals, physicians and community, Senior Services offers programs and support to help seniors and their families live better and continues to develop and expand those services for the future.

Senior Services includes:

  • Seniority
  • Home Connections
  • Navigator
  • Centers for Advanced Quality Outcomes (CAQO)
  • GEROS Medical Group
  • TriHealth Physician Partners Geriatric Medicine
  • TriHealth Senior Services Geriatric Pharmacy

In addition, Bethesda North has earned the designation of Nurses Improving Care for Health System Elders - or NICHE. Through a collaboration with Senior Services, the hospital has plans to add features to its emergency department to make it a Geriatric-Friendly ED with the capability to better serve the geriatric population.

Helping Seniors Maintain Active Lives

Through Seniority, TriHealth helps older adults maintain healthier, more active lives by hosting special events, exclusive classes with expert health education and information and travel opportunities. In addition, this free program rewards members with valuable discounts on health-related products and services. For patients trying to maintain independent living, TriHealth offers a personal emergency response system through TriHealth Home Connections.

Connecting Seniors to Quality Care Options

If complicated medical issues do arise, TriHealth is there to support patients and their families through referral programs such as Navigator and Centers for Advanced Quality Outcomes (CAQO). Navigator staff help connect patients and families to a network of senior care options, both within TriHealth and the community. CAQO offers referrals to a network of quality post-acute care facilities.

Providing Specialized Health Care

In addition to its support programs, TriHealth recently announced the opening of TriHealth Physician Partners Geriatric Medicine in Norwood and TriHealth Senior Services Geriatric Pharmacy. These care providers will be solely focused on the elderly and meeting their needs by offering the specialized care that comes with aging and complex medical issues.

And through GEROS Medical Group, TriHealth is able to provide quality primary care to residents of more than 60 long-term care facilities throughout Greater Cincinnati. GEROS physicians enable continuity of care by communicating to TriHealth hospitalists should hospitalization become necessary.
