LifeStyles representatives will be available in the Bethesda Oak cafeteria on the following dates to answer questions and assist with membership form completion. They'll also have physician consent forms for those who need them.

• Monday, June 24, 11 a.m. to 1 p.m.
• Tuesday, June 25, 11 a.m. to 2 p.m.
• Wednesday, June 26, noon to 3 p.m.
• Thursday, June 27, 10:30 a.m. to noon
