Ensure your student-athlete is in top physical condition for the 2014-15 school sports season.

From 8 to 11 a.m. Saturday, July 19, the Orthopedic and Spine Institute will offer school pre-participation physicals for student-athletes in grades seven through 12 at the TriHealth Fitness and Health Pavilion. The cost per athlete is $20 with $10 returned to the athlete's school for a voucher to purchase sports medicine supplies.

Not appointment is necessary. Physicals will be given in the sports arena on the first floor of the Pavilion, 6200 Pfeiffer Road.

Additional information:

• Students must wear shorts.
• Students must bring a completed Ohio State Physical Form, signed by a parent or guardian. The form is available from the school's athletic office or at ohsaa.org.
• Only cash or checks (no insurance or credit) will be accepted. Make checks payable to Good Samaritan Hospital.
