TriHealth's new Incident Reporting Information System, IRIS, went live on July 1! 

This organization-wide system allows associates, physicians,  and leaders to report and manage information for both incidents and feedback. Incidents are unexpected events, or near misses (previously known as "safety events"). Feedback is a compliment or complaint reported by our customers or patients.

IRIS is easily accessible on the LinkNet from any page: it's in the top right-hand corner above the search bar and under Quick Links in the main menu. In the new future, a link to IRIS will also be available in TriHealth Connect (EPIC). From the IRIS page on LinkNet, users of the system can log in using their TriHealth network login ID and password, or submit an anonymous report by selecting the Incident Form or Feedback Form.

With this one-stop, user-friendly incident and feedback reporting tool, our system looks to increase reports to help identify patient safety and customer service issues. By reporting actual and near-miss incidents, you are initiating the first step toward improving the quality of care and safety within our organization. With a culture of continuous improvement, our system can meet our goal of eliminating errors, promoting patient and workplace safety and providing the highest quality care.

 For TriHealth, IRIS replaces a number of applications:

  • Quantros system (Patient Safety)
  • Respond system (Patient Relations)
  • Stars system (Risk Management)
  • Current databases (Security)

In the near future, additional modules in the IRIS system will go live. Look for more information on LinkNet in the coming months. Modules will impact:

  • Claims - Risk Management activities
  • Safety Alerts - Recalls, safety notices
  • Hot Spots - Alerts based on set criteria

  • For questions relating to use of IRIS, call the Patient Safety Hotline at 569-5432.

  • For technical issues, call the Help Desk at 513 569 5100.

  • There are Help Links within the IRIS system.
  • Additional training is available upon request: Call Patient Safety for one-on-one or group training.

For Urgent Needs 

  • For a serious safety concern, call Risk Management at 513 569 4051 or the hospital operator after hours.
  • For a customer service need, call Patient Relations at Bethesda North, 513 865 1115, or Good Samaritan, 513 862 2528, if the issue demands immediate attention.
  • For security issues, contact the Security Department.
