In the five months since we introduced a defined list of items on the Office Depot ordering site, a team has met weekly to carefully consider nearly 650 requests for additions that have been submitted. The team assesses these requests based what we need to deliver quality, safety and service to our patients.

As a result, many changes have been made. We feel the formulary now offers what the vast majority of staff needs to work effectively and efficiently.

The team will continue meeting to discuss requests. But now, requests for changes to that list must come from directors or above. Requests by directors can be submitted here: Office Supply Request Form. Please contact your director if you have a need for office supplies that are not currently available.  Decisions will be communicated within two weeks after the request is submitted.
