TriHealth Women's Services is pleased to announce the launch of their Nurse Navigation program.
What is Nurse Navigation?
TriHealth Women's Services Nurse Navigation Program is designed to enhance coordination, collaboration, and access of care across the Women's Service Line and throughout TriHealth. The goal of this program is to provide dedicated support and guidance throughout a patient's health care journey, simplifying the steps towards a successful path to the right care and better outcomes. Nurse Navigation is committed to supporting practices and providers by facilitating referrals, supporting care recommendations, addressing common patient concerns and questions, and serving as a vital partner and resource for all.
Nurse Navigation is a complimentary and confidential service available to external, new to care, or established patients.
Nurse Navigation Services include:
- Targeted support for key transitions in care including adolescent gynecology, menopause, and specialty services
- Assisting patients with finding the provider or practice that best suits their individual needs
- Provide introductory education to OB/GYN care, preventative screenings, or health services
- Prepare for an upcoming visit or assist with barriers to care
- Facilitate referrals within Women's Services and across TH Service Lines/Specialties
Meet the Nurse Navigator:
Rebecca Brown, BSN, RN, RNC-OB, C-EFM is a Registered Nurse for serving patients and their families in acute care and outpatient clinical settings. Her nursing experience includes critical and progressive care, cardiology and vascular health, labor and delivery, and prenatal care with a focus on High Risk Obstetrics.
