Hospice of Cincinnati, in collaboration with the American Heart Association and the National Partnership for Healthcare and Hospice Innovation, introduces the HOC Advanced Cardiac Care Program to address one of the nation’s largest killers of Americans: heart disease.
This program actively manages clinically complex cardiac patients at end of life to prevent hospital readmissions, decrease hospital readmission rates, and decrease hospital lengths of stay. The Hospice of Cincinnati team has undergone specialized cardiac education and training, and patients and family members enrolled in this program will receive education tools to prevent symptom exacerbation and improve quality of life while receiving care in their home. All patients referred to Hospice of Cincinnati, with a primary cardiac diagnosis, will be automatically enrolled in the HOC Advanced Cardiac Care Program upon admission.
