Due to the COVID pandemic, the two HARP locations were merged into one location at the Bethesda Family Practice Center at University Station in Norwood. The program continues to be run by the hospitalists with Dr. James Konerman serving as the anchor physician. The HARP program continues to serve three main functions:
- Post-hospitalization follow up: patients with no PCP who are discharged from the hospital or emergency room will follow up with the HARP clinic for appropriate post-discharge care. This includes initial medical follow up and helping the patient get established with a Primary Care Provider.
- Paramedicine Program: this program is in collaboration with Miami Township Paramedicine and is a pilot program designed to help keep patients with recurring low-acuity needs out of the ERs and hospitals. The HARP team works with the paramedics who make home visits to help manage these patients’ medical needs and identify safety issues.
- Housecall program: this initiative is for patients who are discharged from the hospital and have significant barriers preventing them from being able to leave their home and see their primary care physician for follow up.

You can also send a Paramedicine referral directly to the Paramedic James Petry via Voalte.
