Due to the COVID-19 pandemic, the two HARP (High Risk Readmission Prevention) locations were merged into one location at the Bethesda Family Practice Center located at University Station in Norwood near Xavier University.

A patient without a Primary Care Provider (PCP) who is discharged from the hospital or emergency room can be scheduled for a follow up at the Bethesda Family Practice. The service of HARP will continue to provide initial medical follow up and help the patient establish with a PCP. Dr. James Konerman and the Family Medicine Residents lead this resource for our patients.

Additionally, Dr. Konerman will continue to lead the Paramedicine Program from the Bethesda Family Practice Center. The Miami Township Paramedicine pilot program was developed when it was found that a small number of their patient population were responsible for a large number of squad runs. Many of these patients had non-emergency problems. The Paramedics will make home visits
to help the patient manage their medical care and identify safety issues. Referrals are made by care coordinators or PCPs.
