Fulfilling our sacred mission to improve the health status of the people we serve in today’s rapidly evolving market requires us to accelerate our population health work. Over the past three years, we’ve worked hard to establish TriHealth as the leader in population health – both regionally and nationally.   And while we have made encouraging progress, our work is far from complete.

To further propel our population health journey, I’m pleased to announce that Terri Hanlon Bremer, MSN, RN, Vice President of Employer Solutions and Corporate Health, will take on the senior leadership role for Population Health. Terri is an inspiring, values-centered servant leader with a proven track record of innovation and stellar performance. Throughout her career at TriHealth, she has shown ability to successfully manage complex projects and functions involving multiple disciplines and stakeholders. Her clinical background as a registered nurse, combined with a highly successful business career leading Corporate Health, will enable Terri to lead our population health work through the lens of a caregiver.

Over the past few years, we’ve put the essential foundation in place to enable our system to manage the health of populations of patients, and the market is taking notice. For example, we have:

  • Increased our values-based patient population from 5,000 to more than 266,000 members of our community, and the number continues to grow each month.
  • Developed innovative payment models with payer partners to reward TriHealth and our providers for delivering on the triple aim of better care, better health, better value.
  • Been recognized by Anthem as one of the Top Accountable Care Organizations (ACO) in the nation, and the #1 ACO in Ohio.
  • Achieved NCQA certification for Care Management, one of only eight health systems nationally to receive this recognition.
  • Forged an innovative partnership with Anthem Blue Cross (the nation’s second health largest insurance company) to bring a more affordable insurance product to Cincinnati’s employers with TriHealth as the exclusive health care provider.

In Terri’s new and expanded role, she will be responsible for building on this impressive foundation to accelerate our journey to achieve value-based care’s triple aim of better, more affordable care for every individual we serve. Specifically, she will lead our key population health functions, including analytics, ambulatory care management, care model innovation, and disease management, in collaboration with the TriHealth Provider Network—the PHO.
