2019–20 School Pre-participation Physicals
Make sure your student-athlete is in top performance shape for the 2019–20 sports season.

TriHealth Fitness & Health Pavilion
Sports Arena, First Floor

Student-athletes entering grades 7–12
- No appointment necessary.
- Students must wear shorts.
- Students must bring a completed Ohio State Physical Form signed by a parent/guardian. This form is available from the school’s athletic office or at ohsaa.org.
Cost: $20 per athlete. $10 of each fee is returned to the student’s school for a voucher to purchase sports medicine supplies.
- No insurance accepted.
- Cash or check only. Please make checks payable to Good Samaritan Hospital.

I work in one of the TriHealth Pediatric offices and we were not notified of this. Please make sure to notify the pediatric and family practice offices in the future!
