The HARP (Hospital Aftercare and Recovery Program) Clinic is changing on Wednesday, July 20th to serve the TriHealth System better.

The Hospital and Emergency Room Care Coordinators will attempt to schedule patients who have 'NO Primary Care Provider (No-PCP)' at the time of their visit with a Primary Care Provider while the patient is still at their facility.

This new program will take some time to get phased into place.

Patients that need to see a provider quickly within 2 weeks and can’t get scheduled in that time period may be referred to one of these locations:

  1. 'No PCP' patients coming out of Good Samaritan Hospital Clifton or Good Samaritan Hospital Western Ridge should be referred to HARP at the FACULTY MEDICAL CENTER. The scheduler or patient can call 513 862 1800 to get scheduled there.
  2. 'No PCP' patients coming out of Bethesda North Hospital, Bethesda Butler Hospital, or Bethesda Arrowsprings should be referred to the PC HARP PAVILION (Dr. Konerman) which is at 6200 Pfeiffer Rd Suite 330. The scheduler can schedule in Epic. The scheduler or patient can also call 513 853 9380 to get scheduled.

We are hopeful that this new design will provide for improved care for the discharged patients and reduce readmissions.
